How do I know when it's time to put my old girl down?

My Ginger is almost 15 years old.  She is a chocolate Lab who has given me so much joy.  She is still eating but, she has lost control of her bowels.  She walks, real slow, falling most of the time or, accidentally being knocked down.  She has such a hard time getting up or laying back down.  We help her up when we are in the room to see her struggle.  I don't think that she's got any quality of life laying around, pooping herself, and falling down but, who am I to make this big decision for my old friend?  Please help me in my attempt to do the right thing by her.
